A Qantas A380 flew a round trip between Sydney and Dallas-Fort Worth with passengers aboard while a 33-centimeter (about 13-inch) battery-powered maintenance work light sat forgotten inside its left wing, Australia’s Transport Safety Bureau (ATSB (Australian Transport Safety Bureau)) has found. Two workers had used the light to see inside the wing while replacing an air-conditioning sensor during maintenance at Sydney Airport on January 7, 2026. The aircraft was cleared for service and completed both legs of the trip. After it returned to Sydney, the light was found to be missing. It was recovered from the wing about two days later, on January 9, 2026.

What ATSB found
No one was hurt, and the aircraft was not damaged. ATSB said the light did not affect how the plane operated, but the agency also said foreign objects left behind during maintenance generally pose “a significant risk to the safe operation of an aircraft.”
ATSB’s investigation, released Aug. 17, 2026, traced the miss to a breakdown in Qantas’s tool-accountability process. Three separate safety checks were supposed to catch anything left behind after the job was finished, and all three missed the light:
- An end-of-maintenance inspection for foreign objects
- A check to confirm every tool had been returned
- A final count of unreturned tools at the end of the shift
Investigators said the space inside the wing was confined, poorly lit and hot, which likely made the light harder to spot. They also found that Qantas’s maintenance-tracking system did not automatically alert the engineer who certified the aircraft as fit to fly that a tool was still unaccounted for.
ATSB Chief Commissioner Angus Mitchell said: “Even well-established processes can be vulnerable when they rely solely on consistent human performance, particularly during maintenance tasks which are often conducted in challenging environments and under operational pressures.” He added that tool-control systems and procedures “must account for this variability,” with safeguards built in to catch and correct mistakes before they matter.
Qantas’s response
Qantas reported the incident to ATSB voluntarily. The airline says it has since tightened how it tracks tools during maintenance, telling ATSB: “Since the event, we have strengthened our tool-control procedures, including introducing a mandatory pre-clearance check to confirm all tooling is accounted for following maintenance.” Qantas also said it has established a tooling working group.
A second, earlier incident
This is not the first time a maintenance tool has gone missing inside a Qantas A380. In a separate, earlier case, a 1.25-meter (about 4 feet) nylon tool used during an internal engine check was left inside the engine of another Qantas A380 during maintenance in Los Angeles in early December 2023. The aircraft wasn’t due back in for its next scheduled maintenance check until weeks later, and by the time engineers found the tool, on January 1, 2024, the plane had flown roughly 294 hours on multiple international trips with the tool still wedged inside. The tool was later found deformed by the engine’s airflow, but the engine itself was not damaged. ATSB’s report on that incident, published in November 2024, described it as the result of “a cascade of errors, of processes that weren’t followed.” Qantas’s then-head of safety, Mark Cameron, said the airline worked with its maintenance teams to review training and processes afterward.
Both incidents ended without injury or damage. Qantas says it has since strengthened its tool-control procedures.
